At first we were really skeptical about everything that Harris CCM was saying because it all sounded too good to be true. But after going through the implementation process, and working with the team, and launching the program, we are pleasantly surprised at how easy it has been to manage and grow the program.

Since we have started using Harris CCM Services, our patient just respond quicker and it has saved so much time versus playing phone tag. We can better monitor a patient’s blood pressure, weight and other symptoms. Harris CCM has helped us improved our patient’s outcomes.

The chronic care management program allows our providers to proactively manage, chronically ill patients outside the normal office setting thereby providing improved patient care, higher patient satisfaction as well as reimbursed for those services.
