NURSE CHRONIC CARE COORDINATOR
The position of the Nurse Chronic Care Coordinator, Remote will perform telephonic encounters with patients on behalf of our partners and develops detailed care plans. This begins as an Independent Contractor position, but offers the potential to reach full-time employment (with employee benefits – contact us to learn more!).
Harris CCM is seeking nurses to work part-time from their home office while complying with HIPAA privacy laws. You will set your own hours and will not be held to a daily work hour schedule. Harris CCM wants its team members to have the flexibility to balance their work-life with their home life. Part-time team members will typically need to dedicate an average of 20 hours per week to care for their assigned patients. This unique business model allows you to choose what days and what hours of the day you dedicate to caring for your patients.

Position Type & Expected Hour of Work
This position is part-time. The Care Coordinator will be assigned a patient panel based on skill and efficiency level and is expected to reach a minimum of 150 encounters per calendar month for Part-Time team members and a minimum of 250 encounters per calendar month for Full-Time team members. Care Coordinators will be expected to complete encounters on 90 percent of the patients they are assigned.
PAY STRUCTURE
Harris CCM utilizes a productivity based pay structure and pays $10 to $16 per completed patient encounter. A patient encounter will take a minimum of 20 minutes (time is cumulative).
REQUIRED EDUCATION AND EXPERIENCE
Active Compact RN/LPN/LVN license in state of residence. A minimum of two (2) years of clinical experience in a Med/Surg, Case Management, and/or home health care.
Hands-on experience with Electronic Medical Records as well as knowledge of Windows desktop and applications.
Ability to exercise initiative, judgment, organization, problem-solving, and decision- making Skilled in using various computer programs (If you don’t love computers, you won’t love this position!) Capable of organizing work to achieve daily/ weekly goals and objectives.
Excellent verbal, written and listening skills are a must.
